
Our patient-focused primary care is delivered by skilled, experienced professionals focused on your overall well-being, not just a single problem or illness. For adults and qualifying seniors, we are here to answer your health questions, provide preventive care, order tests, prescribe medications, help you manage chronic conditions and coordinate specialty care when needed. We are here to help provide the tools and support our community needs to live healthier lives. In other words, primary care is your first line of defense against day-to-day illnesses and chronic conditions. Think of it as an ongoing maintenance program for your body, your health and complete well-being.
So whether you have the flu, you need a physical, or your parent needs help managing chronic diseases, we’re here for you. We are accepting new patients from 18 years old and up. If seniors are not eligible for Medicare, they are welcome to use all of the services at the clinic. If seniors have Medicare without Part D, the clinic can help them access expensive brand name medications.
And if your primary care provider determines you need an evaluation by an outside specialist, like a cardiologist, neurologist or other specialists, we’ll refer you to one. In-house referrals are also available with a referral from your Primary Care Provider.
